At Physio Mechanics Ltd., we understand how important it is to protect your privacy. We fully comply with The General Data Protection Regulation (GDPR), which comes into effect from 25th May 2018, on how we collect, store and use your personal information. We are regulated by the Health and Care Professions Council (HCPC) and are members of the Chartered Society of Physiotherapy (CSP) - we adhere to their standards and guidance on record-keeping and data protection. We are registered with the Information Commissioner's Office (ICO) - registration number ZA309208.

Information we collect:

  • Personal details - eg. name, address, date of birth.

    • This is used to identify you and ensure security and confidentiality.

    • For example, when we are accessing your file or if you have requested your data, we can ensure you are who you say you are and there can be no misfiling or mistakes.

    • This is also used to ensure accurate accounting and invoicing - any card/payment details are held securely by our payment processing system Square.

  • Contact information - eg. email address and phone numbers​.

    • Used to send you appointment confirmations, reminders, exercises and information about our services and offers - these are only sent with your consent and you may unsubscribe at any time - contact us.

    • May be used to contact you if changes are required to scheduled appointments eg. due to weather or staff absence.

  • Medical information - eg. details of your injury, any current and previous illnesses, medications, surgeries or treatment​. 

    • To help us make an accurate diagnosis of your injury or movement problem.

    • It also helps us decide which treatments are safe and appropriate for you and the advice to give you.

  • Cookies â€‹

    • This website creates small files that are stored on a user's computer. You can check and delete any cookies you don't want on your computer from your internet browser. You also have the option to disable cookies in your internet browser so they are not received.

    • Cookies are used to collect anonymous details about you - the country you are in, the time you access and spend on the website and how you accessed it (eg. by desktop or mobile, from search engines or direct links etc).

We send out a variety of communications regarding booked appointments:

  • Appointment confirmation emails

  • Appointment reminders by email or text message

  • Exercises by email

If you wish to be kept up to date with information from Physio Mechanics Ltd. please contact us to subscribe to email newsletters.

You will be asked for consent for these communications during your first appointment with Physio Mechanics Ltd. You may unsubscribe from these and any communications at any time.

Information storage and sharing:
 

To enable Physio Mechanics Ltd. to run a professional service, we use third party online programmes for the collection and storage of information. These are secure sites which are password protected. Paper records are stored in a locked cabinet only until it is entered into the patient management software then destroyed. ​

Information collected is not shared with other third parties (eg. your GP, a consultant or another therapist) unless following discussion with you and with your consent unless where we are legally required to do so.

Health records for adults must be stored for 6 years after their last consultation. For children, this is until their 25th birthday (26th birthday if they are 17 at the time of their last consultation). Maternity records are kept for 25 years. After these timescales, records are destroyed.

Your right to access:

 

You may request access to any and all information we hold about you.

 

You may also request we correct any information held about you. Please note that health records are completed and finalised within 24 hours of each appointment and legally cannot be modified. If there are any inaccuracies you wish corrected a note may be made in your file to reflect this. 

 

We will accept requests in any form (written, email or verbally) and once we have confirmed your identity will provide you with a copy. This will usually be in electronic format, but a paper copy may be requested. The GDPR requires we complete this request within one month - contact us.

 

 

Changes to this policy:
 

Physio Mechanics Ltd. reserves the right to modify or update this Privacy Policy in line with current legislation and will be available on this page immediately.
